Please read the following liability waiver and release before attending any services in-person. We assume that by proceeding to register yourself and attend, you have agreed with the items listed on the release.

This Liability Waiver and Release is entered into between the participant (“Participant”) and Seattle Koyasan Buddhist Temple (“Seattle Koyasan”).

ASSUMPTION OF RISK – I acknowledge that practicing any physical exercise, class, or activity may result in accident or injury. I voluntarily assume all risks associated with participation, including use of any equipment or props provided on the premises.

PHYSICAL CONDITION – I affirm that I am in good health and suffer from no known physical impairment that would limit my participation in any services offered at the temple. I understand that I have been informed of the need for a physician’s approval prior to participating in exercise activities and that a physical examination and consultation with my physician have been recommended.

MEDICAL & MENTAL HEALTH DISCLAIMER – I acknowledge that Seattle Koyasan's services are provided for religious or spiritual purposes. I understand this is a non-healthcare, non-clinical relationship. No medical services, mental health care, counseling, diagnosis, or treatment of physical or psychological conditions will be provided. I further understand that all activities are undertaken at my own risk and discretion, and I am responsible for consulting with my own healthcare or mental health provider regarding any medical or psychological concerns.

RELEASE OF LIABILITY – I specifically agree that Seattle Koyasan, its officers, employees, agents, and Instructor shall not be liable for any claim, demand, or cause of action of any kind — including death, personal injury, property damage, or loss — resulting from or related to my participation in Pilates classes or use of the facilities. I agree to hold Seattle Koyasan and the Instructor harmless from such claims.

MEDIA AGREEMENT – I understand that photographs, videos, recordings, or testimonials may be taken during classes or events. By proceeding with this registration and by attending the services, I grant permission to Seattle Koyasan to use such media for informational or promotional purposes. I acknowledge that no compensation will be provided for such use, and that all media becomes the property of Seattle Koyasan.

ACKNOWLEDGMENT – I have read this release and waiver of liability, fully understand its contents, and voluntarily agree to the terms and conditions stated above. I confirm that my participation is voluntary and that I may stop at any time.
